Is Cow’s Milk Suitable for Infants?

As parents, one of the most common questions we hear in the clinic is:
“Doctor, can I start giving cow’s milk to my baby?”
The short answer is:
👉 Cow’s milk is NOT suitable as the main milk feed for babies under 1 year of age.
Let’s understand why.
Why Cow’s Milk Is Not Recommended Before 1 Year
1️⃣ It Can Cause Iron Deficiency
Cow’s milk:
- Contains very little iron
- Can interfere with iron absorption
- May cause small amounts of intestinal blood loss in some infants
This increases the risk of iron deficiency anemia, which can affect brain development.
2️⃣ It Is Harder on Baby’s Kidneys
Compared to breast milk, cow’s milk:
- Has higher protein content
- Has higher salt (sodium, potassium) load
A baby’s kidneys are still immature and may struggle, especially during:
- Fever
- Diarrhea
- Dehydration
3️⃣ Different Protein Composition
Cow’s milk contains more casein, which is harder to digest for infants.
Human milk is naturally designed for human babies.
What Should Babies Drink Instead?
✔ 0–6 months:
Exclusive breastfeeding is best.
✔ 6–12 months:
Continue breastfeeding.
If breastfeeding is not possible, use iron-fortified infant formula.
When Can Cow’s Milk Be Started?
✅ After 12 months of age
- Use whole milk (not toned or skimmed)
- Limit to about 400–500 ml per day
- Continue offering a balanced solid diet
